Dear Northwest Allen County School Parent/Guardian,

In partnership with Super Shot, we will be providing an optional in-school Flu Mist this Fall for NACS students on Thursday September 29, 2022. When students are immunized against the flu it keeps them healthy, prevents absenteeism, and keeps our entire school community healthy. 

Most health insurance plans cover the cost of vaccines. Super Shot is required to bill insurance and will run eligibility (not a guarantee of payment) per the completed consent. Super Shot is in-network with Medicaid and most commercial insurance plans. For commercial plans, please contact your insurance for coverage details. 

Please review the attached consent form to make sure your child meets the standards to receive the flu vaccine. If your child is not eligible to receive the flu mist in school, you will be notified of the recommendation to check with your child’s health care provider. 

Please register with Super Shot online by Friday Sept. 16, 2022 for your child to receive a flu mist at school. Attached is the Vaccine Information Sheet, for your review and reference.
